Vein Expert: New Studies Show Therapies Safer, More Effective

New therapies — including a treatment just made available in August — are leading to more effective vein treatments, Mitchel P. Goldman, M.D, a leading expert on vein treatments, told a gathering of skin and beauty experts recently.

Working with other dermatologic surgeons, Goldman has studied the safety, effectiveness and satisfaction ratings of foam sclerotherapy for veins on the legs, hands and chest as well as the use of a long-pulse Nd:YAG dynamically cooled laser to treat facial veins.

His results are just now being released, in peer-reviewed scientific journals.

Goldman, president of the American Society for Dermatologic Surgery, spoke recently at the late-September ASDS “Skintelligence” presentation in New York City. During Skintelligence, he and several other dermatologic surgeons outlined innovative procedures and new trends in cosmetic and medical skin treatments.

Foam sclerotherapy involves mixing an injectable sclerosant solution with air or carbon dioxide so the resulting foam increases the drug’s surface area in the veins, displacing the blood and causing eventual closure and disappearance of the veins.

Goldman was among the first in the country to perform foam sclerotherapy for leg and hand veins and then teach others the procedure. He also has written extensively about the treatment.

Treatment of large leg varicose veins has taken a big step forward with the debut of Varithena, which combines Polidocanol and a carbon dioxide mix to create a new injectable foam. “Foam treatments have come a long way,” said Goldman.

Varithena, by BTG, became commercially available in August after receiving approval from the U.S. Food and Drug Administration in late 2013. It can be used to treat veins in the great saphenous vein system, which runs the entire length of the leg. Goldman conducted clinical trials on Varithena before FDA approval.

While some patients may regard veins as essentially a cosmetic concern, Goldman said leg veins can cause health problems. Fifty percent of patients with varicose and other leg vein problems experience blood clots or ulceration. Varicose veins also can cause night cramps, restless legs, pain and other issues, in addition to cosmetic concerns. An estimated 30 million-plus Americans ages 18 to 70 have varicose veins, he said.

A survey of 221 patients who participated in the final clinical trial for Varithena shows success with the new treatment, Goldman said.

At one year post-treatment, the patients showed consistent improvements in appearance; disease severity; and symptoms such as heaviness, aching, swelling, throbbing and itching, Goldman said. Both physicians and the patients themselves assessed how the leg looked.

“I have no doubt Varithena will be an important addition for physicians’ abilities to treat leg veins successfully,” Goldman said.

Goldman said prominent hand reticular veins – the dilated blue and green veins just beneath the skin’s surface – bother many of his patients. Though he has used foam sclerotherapy effectively on veins in the hand for years, “The treatment is not widely used yet and the literature in terms of side effects and treatment outcomes is somewhat lacking.”

He and Drs. Anne Marie Tremaine and Daniel P. Friedmann reported in August the results of a retrospective review for both effectiveness and safety of patients whose hands had been treated with foam therapy between 2003 to 2012.

Patients representing 30 of the 38 treated hands reported themselves “very satisfied” with the results, with only one expressing dissatisfaction. Eighty-one percent said they would definitely consider repeat treatments if necessary.

Adverse events following treatment were rare and mild, Goldman said. The most common side effect was blood coagulation, as had been seen in earlier studies, followed by mild pain, redness, bruising and swelling.

“Based on our experience, foam sclerotherapy seems to be a safe and effective means for treating prominent reticular veins on the back of the hands,” he said. “Patients had excellent long-term satisfaction and experienced far less adverse events compared with sclerotherapy of leg veins.”

Goldman – working with Friedmann and Andres Cruz, M.D. – similarly found foam sclerotherapy effective for treating blue and green surface veins appearing on the chest.

The three dermatologic surgeons followed up with 12 patients, all female, who had undergone a total of 16 procedures. “Patients reported substantial long-term efficacy and satisfaction scores, with few adverse events,” Goldman said. “Though we believe further studies are needed, initial indications are that foam sclerotherapy is a very viable treatment option.”

Facial Vein Treatments

While facial surface veins cause no harm, many patients find the visible blue and green surface veins distressing, Goldman said. Yet there is no true gold standard for treating what patients regard as unsightly veins near the eye socket or temples, he said.

Through surveying former patients treated with a dynamically cooled laser, he and Ana Marie Liolios, M.D., conclude the procedure “is a safe and effective treatment for facial reticular veins with minimal side effects,” said Goldman. The non-invasive treatment utilizes the dynamically cooled, variable spot size 1064 Nd:YAG laser, known as the CoolTouch Varia.

Other treatments – including intense pulsed light therapy and a variety of other laser procedures – are popular, but they do not specifically target larger surface veins, Goldman said. Moreover, many of the treatments are not without risk.

“There also is hesitation using sclerosing, or foaming, injectable agents because of the proximity of the veins near the eyes,” he said.

Of the 40 patients interviewed by Goldman and Liolios, 30 reported themselves “very satisfied” and their improvement ratings averaged 81.2 percent. The most common side effect was mild swelling, and half reported no adverse effects at all.

One patient reported the veins reappeared after six to nine months. This patient had a repeat treatment at a slightly higher energy level with good results, Goldman said.

The dynamically cooled, variable spot size 1064 Nd:YAG laser is a safe and effective treatment for facial reticular veins with minimal side effects, he and Liolios concluded. The results are usually immediate and, based on a long-term follow-up, tend to be long-lasting, Goldman said.

The Pesticide Safety Education Program Reaches a 50-Year Milestone

Today scientists with the Weed Science Society of America (WSSA) joined with the American Phytopathological Society (APS) and the Entomological Society of America (ESA) to recognize the 50-year anniversary of the national Land-Grant University Pesticide Safety Education Program. Although the program has evolved over the past 50 years, it remains the focal point for pesticide safety education throughout the United States.

The Pesticide Safety Education Program (PSEP) had its genesis in 1964 to enhance pesticide label compliance and to develop the first training manuals. In the early years, the program was under the direction of each Land-Grant University’s Cooperative Extension Program and was supported by the U.S. Department of Agriculture (USDA). In 1970, USDA passed the safe-use education torch to the newly created U.S. Environmental Protection Agency (EPA), which concentrated especially on safety issues on the farm and in other occupations.

In 1978, EPA classified the first 12 restricted-use pesticides (RUPs). Applicators were required to demonstrate competency to apply RUPs, and Pesticide Applicator Training (as PSEP was then called) served as the primary developer and deliverer to inform and educate on safe pesticide use.

The reach of the Pesticide Safety Education Program has expanded greatly over the years. There are many more RUPs, and many states now require whole categories of users to be certified, even if they do not apply RUPs. Examples include certification of hired applicators, public employees and those treating schools or aquatic environments. In fact, an estimated 40% of certified applicators in the U.S. today do not apply RUPs.

Though it once focused predominantly on the education of applicators controlling agricultural pests, PSEP now teaches applicators working in urban, natural, industrial and other settings. It provides training to those who control weeds, insects, disease-causing organisms, rodents and other pests in forests, structures, turf, ornamentals, rights-of-way, aquatic areas, and other important and sometimes unique “sites.” These include food manufacturing and processing establishments, interior plantscapes, pet grooming, pools, public health, seed treatment, sewers, water sanitation, wood preservation and more. In addition, PSEP impacts more than one million pesticide users in the general public who apply pesticides in their homes and on their lawns, gardens, ornamentals and pets.

Last year, approximately 900,000 certified applicators in the U.S. applied pesticides or supervised their use. Many more individuals who did not require certification sold, transported, stored, mixed, applied, disposed or were otherwise involved in the life-cycle management of pesticides. To reach all these audiences, PSEP and its not-for-profit partners provided in-person and on-line training sessions, distance education, manuals, brochures, presentations and videos. These resources promote safe handling of pesticides and protection of applicators, workers, the general public, beneficial organisms and the environment.

Everyone benefits from a strong national Pesticide Safety Education Program – the general public, the registrants whose products’ availability depends on safe use, the applicators who must be competent in the safe use of pesticides, the expanded network of trainers educated by PSEP and the regulatory agencies that enforce the law.

The recognition this program deserves is often muted, due to the increasing number of organizations and initiatives that erroneously equate pesticide safety education with promoting pesticide use. On its 50th anniversary, the WSSA, APS and ESA salute the Pesticide Safety Education Program in the Land-Grant Universities and in the territories for its many efforts to protect human health and the environment, as society continues its ongoing battle against pests.

As the latest phase of the U.S. Department of Agriculture’s (USDA) strong school nutrition standards take effect, the Alliance for a Healthier Generation will honor 250 schools for becoming healthier places. School leaders will receive National Healthy Schools Awards at the Alliance for a Healthier Generation’s Leaders Summit, September 12 – 13, in Washington, D.C. President Bill Clinton will recognize the event guests and award recipients for their efforts in the fight against childhood obesity on Friday evening.

President Clinton also will share data showing how the Alliance paved the way for healthier school meals through a series of agreements brokered with leading school food manufacturers in 2011, prior to federal nutrition standards taking effect. According to preliminary results from a multi-year evaluation, the sale of “Alliance- compliant” healthier school meal products by these leading manufactures—those products with whole grains, lean protein, and zero trans-fat cooking oils; non-fried vegetables; fresh/frozen/canned fruit; and low-fat lunch entrées with reduced total fat, saturated fat, and sodium levels—increased 71 percent between the 2010-11 and 2012-13 school years.

“This data suggests that unhealthy food choices are being replaced by healthy food choices for millions of students across the country,” said President Bill Clinton, co-founder of the Alliance for a Healthier Generation. “The Alliance for a Healthier Generation’s work with schools and food companies proves that with planning and support, manufacturers can make and sell healthier foods, schools will offer it, and kids will eat it.”

School Awardees

Recognized schools are taking advantage of the healthier choices and are leading by example. Working with the Alliance’s Healthy Schools Program, an evidence-based, national initiative that guides schools to create, implement, and sustain healthy environments, schools across the country are making comprehensive healthy changes. One hundred percent of the 250 schools receiving the National Healthy Schools Award:

Meet or exceed updated federal nutrition standards for school breakfast and lunch, which include increased fruits and vegetables, whole grain-rich items and meals lower in saturated fat
Offer school breakfast to their students every day
Met the beverage requirements for the federal Smart Snacks in School standards ahead of the July 1, 2014 requirement
Implement district wellness policies and report progress annually, ahead of the proposed rule
Provide students with at least 60 minutes of physical education per week and ensure activity throughout the school day

The 250 recognized schools, 64 percent of which are high need (40 percent or more of the student population is eligible for free and reduced price lunches), come from 30 states. Each recognized school meets or exceeds stringent standards set by the Alliance for a Healthier Generation for healthy eating and physical activity programs and policies. Schools are eligible for National Healthy Schools Awards at the Bronze, Silver, or Gold level based on the success they achieve and are also eligible to achieve national recognition with Let’s Move! These awardees are a fraction of the total schools enrolled in the Alliance’s Healthy Schools Program across the country. Of all the schools that participate in the Program and have such data (approx. 7,000), 97 percent are meeting USDA school meal regulations and 72 percent are meeting the Smart Snacks in School beverage standards.

“With the support of the Alliance’s Healthy Schools Program, the schools we’re honoring today are promoting healthy eating and physical activity policies and practices that are essential to building a national Culture of Health,” said Dr. John Lumpkin, senior vice president for the Robert Wood Johnson Foundation, a primary funder of the Healthy Schools Program. “Their leadership and commitment are making a critical difference in helping to reverse the nation’s epidemic of childhood obesity, and giving kids the right start in leading long, healthy lives.”

“Childhood obesity can carry severe medical consequences for our kids, which is why I’m particularly excited about the accomplishments of these schools. With support from the Alliance, they have inspired staff and students to eat healthier foods and be more physically active,” said Dr. Elliott Antman, president of the American Heart Association, which co-founded the Alliance for a Healthier Generation with the Clinton Foundation.

Industry Data

Through Alliance agreements signed in 2011, prior to the implementation of federal nutrition standards, leading school food manufacturers pledged to price healthier items at rates no higher than comparable “non-compliant” items, to shift sales toward products that meet the Alliance’s science-based nutritional criteria, and to shift marketing efforts toward these healthier products.

Now, three years later, at the 2014 Leaders Summit, President Clinton will announce preliminary data regarding the Alliance’s work with school food manufacturers to improve the nutrition of school meals. The data was monitored and validated by Keybridge, a Washington, D.C.-based economic and public policy consulting firm. The data illustrates changes being made by industry to bring healthier and affordable foods into school lunchrooms. In fact, total sales of healthier, Alliance-compliant products to participating schools increased by $ 130 million, or 71 percent, between the 2010-11 and 2012-13 school years. Alliance-compliant product sales represented nearly a third of all school food sales by these participating manufacturers during the 2012-13 school year.

Representing the majority of the school meals market for proteins and potato products and sizeable shares in most other product categories, participating manufacturers include AdvancePierre Foods, Aryzta, Asian Food Solutions, Bake Crafters, ConAgra Foods, High Liner Foods, JTM Food Group, McCain Foods USA, Rich Products Corporation, Tasty Brands, Trident Seafoods, and Tyson Foods.

Community and Business Awardees

This year, the Alliance is awarding Healthier Generation Hero Awards to community and business leaders who have made extraordinary efforts to tackle the childhood obesity epidemic.

The recipient of the Alliance’s Healthy Out-of-School Time Hero Award is the Wilson Park Unit of the Boys & Girls Clubs of Philadelphia, a community organization that contributes to the healthy development of the young people it works with by focusing on healthy eating and physical activity. The organization replaced unhealthy foods with healthy foods and beverages, found ways to make holiday celebrations healthier, introduced physical activities such as relay races and dancing, and engaged the community in their wellness efforts.

The recipient of the Corporate Hero Award is Sanofi US. Because of Sanofi’s commitment to the Alliance, the organization has been able to implement its approach to school wellness in more than 90 schools in New Jersey, directly benefitting more than 56,000 students. In addition, more than 13,000 children of Sanofi US employees now have access to critical preventive services through the Alliance’s Healthier Generation Benefit, which helps promote the development of life-long healthy habits among children and families.

The Alliance for a Healthier Generation’s Healthy Schools Program helps to create and sustain healthy school environments where students, especially those in greatest need, can learn more and flourish.
